Smoothelin and caldesmon are reliable markers for distinguishing muscularis propria from desmoplasia: a critical distinction for accurate staging colorectal adenocarcinoma.

چکیده
An accurate distinction between deep muscularis propria invasion versus subserosal invasion by colonic adenocarcinoma is essential for the accurate staging of cancer and subsequent optimal patient management. However, problems may arise in pathologic staging when extensive desmoplasia blurs the junction between deep muscularis propria and subserosal fibroadipose tissue. To address this issue, forty-three (43) cases of colonic adenocarcinoma resections from 2007-2009 at The Methodist Hospital in Houston, TX were reviewed. These cases were selected to address possible challenges in differentiating deep muscularis propria invasion from superficial subserosal invasion based on H&E staining alone. Immunohistochemical staining using smooth muscle actin (SMA), smoothelin, and caldesmon were performed on 51 cases: 8 cases of pT1 tumors (used mainly as control); 12 pT2 tumors; and 31 pT3 tumors. All 51 (100%) had diffuse, strong (3+) immunoreactivity for caldesmon and smoothelin in the muscularis propria with a granular cytoplasmic staining pattern. However, the desmoplastic areas of these tumors, composed of spindled fibroblasts and myofibroblasts, showed negative immunostaining for caldesmon and smoothelin (0/35). SMA strongly stained the muscularis propria and weakly (1+) or moderately (2+) stained the spindled fibroblasts in the desmoplastic areas (the latter presumably because of myofibroblastic differentiation). Compared to SMA, caldesmon and smoothelin are more specific stains that allow better delineation of the muscularis propria from the desmoplastic stromal reaction which provides a critical aide for proper staging of colonic adenocarcinoma and subsequent patient care.

Antigenic Profile of Muscularis Mucosae and Muscularis Propria of the Urinary Bladder
Background: Differentiation between the muscularis mucosae (MM) and muscularispropria (MP) of the bladder remains challenging. Objective: To identify MM- and MP-specific antigens that could be of potential value for staging of urothelialcarcinomain a pilot study. Method: The expression of 12 protein antigens in 11 human bladder specimens was examined. There were 5 post radical cystectomy specim...
متن کاملDiagnostic utility of antibody to smoothelin in the distinction of muscularis propria from muscularis mucosae of the urinary bladder: a potential ancillary tool in the pathologic staging of invasive urothelial carcinoma.
Accurate recognition of urinary bladder muscularis propria (MP) invasion by urothelial carcinoma is crucial as it is the critical crossroad between conservative and aggressive management for the patient. It is now widely known that an inconsistent layer of muscularis mucosae (MM) muscle exists in the lamina propria, which can mimic the MP muscle, particularly when hyperplastic, making staging e...
متن کاملValue of histomorphometric tumour thickness and smoothelin for conventional m-classification in early oesophageal adenocarcinoma
AIM To test the validity of tumour thickness measurement in distinguishing between the different infiltration depths, especially when the duplication of muscularis mucosae cannot be demarcated clearly. METHODS We re-evaluated 100 completely embedded Barrett's adenocarcinomas regarding m-classification, maximum tumour thickness, and muscularis mucosae duplication. For validation, smoothelin st...
متن کاملComparison of survival and prognostic factors in patients with gastric adenocarcinoma in T2 and T3.
OBJECTIVE To compare the survival and prognosis after surgical treatment of patients with gastric adenocarcinoma which extends to the muscular layer (T2), and patients whose tumor invades the subserosa (T3). METHODS This was a retrospective study of 122 patients with gastric cancer invading the muscularis propria and subserosa, undergoing surgical treatment from January 1997 to December 2008 ...
متن کاملIs the Use of Cold-Cup Biopsy Necessary at the End of Transurethral Resection of Bladder Tumors?
Bladder cancer is a significant cause of morbidity and mortality today. Histopathological staging [1,2] and adequate tumor clearance is critical in prognosticating and assessing adequacy of treatment. Obtaining detrusor muscles during transurethral resection of bladder tumours (TURBT) is vital for cancer staging [3]. Deep cold-cup biopsy at transurethral resection of bladder tumors (TURBT) is n...
